We are seeking volunteers who are interested in becoming Lay Leaders for the Chronic Disease Self-Management Program (CDSMP).  This is a great opportunity to help improve the health of Delawareans. Medical and non-medical volunteers, with or without a chronic disease are all welcome!   We have 3 training sessions scheduled and listed below. Flyers are attached with additional details

This training is the anchor training for our Self-Management Lay Leader program. An approved application for the lay leader program is required. After completing this 4-day training, you will be eligible to attend Diabetes Lay Leader Training, Chronic Pain Lay Leader Training, and/or Cancer Thriving and Surviving Lay Leader Training

Please help us to fill up the classrooms for all three training sessions! The 4-day anchor trainings will be offered in each county, as follows (must attend all four days for training):

Thursdays & Fridays:  

  • September 12 & 13 AND 19 & 20, 8:30 a.m. – 4:30 p.m. at Thresholds, Inc. (Room A, 20505 DuPont Blvd, Georgetown, DE 19947)
  • September 19 & 20 and 26 & 27, 8:30 a.m. – 4:30 p.m. at Thomas Collins Building (Training Room, 540 S DuPont Hwy, Dover, DE 19901)

Saturdays & Sundays:

  • September 14 & 15 and 21 & 22 8:00 a.m. – 4:00 p.m. at The Chapel at DHSS Herman Holloway Campus (The Chapel on Campus, 1901 N DuPont Highway, New Castle, DE 19720)
