    We are seeking volunteers who are interested in becoming Lay Leaders for the Chronic Disease Self-Management Program (CDSMP).  This is a great opportunity to help improve the health of Delawareans. Medical and non-medical volunteers, with or without a chronic disease are all welcome!   We have 3 training sessions scheduled and listed below. Flyers are attached with additional details

    This training is the anchor training for our Self-Management Lay Leader program. An approved application for the lay leader program is required. After completing this 4-day training, you will be eligible to attend Diabetes Lay Leader Training, Chronic Pain Lay Leader Training, and/or Cancer Thriving and Surviving Lay Leader Training
