Introduction

You do not have to deal with this disease alone. The Diabetes Resource Guide is developed by the Delaware Diabetes Coalition for you. This community of professionals wants you to live a healthier life by assuring you receive care according to the evidence-based standards of care. Use this guide to learn what you can do to manage your diabetes. Additionally, we encourage you to complete the Diabetes Check Card located on the inside flap of the back cover of this Resource Guide. Tear it off and keep it with you. It documents your vital signs for diabetes care so you can compare your results to national standards. Keep this vital signs Diabetes Check Card with you and review it with all providers on your healthcare team to manage your care and live a healthier life with diabetes.

A private, not-for-profit teaching hospital with two campuses and more than 250 Medical-Dental residents and fellows, Christiana Care is recognized as a regional center for excellence in cardiology, cancer and women’s health services. The health system includes a network of primary care physicians, medical and surgical specialists as well as home health care, preventive medicine, and rehabilitation services. Christiana Care is home to Delaware’s only Level I trauma center and includes a Level III neonatal intensive care unit.
