Nicole fullmerNicole Fullmer | Executive Director, Honoring Choices® Delaware

Nicole pours her passion for helping people on their grief journeys into her role at Honoring Choices® Delaware. Since losing her husband to a heart attack at age 43, Nicole has worked tirelessly to connect people to the resources needed for healing and moving forward.  She champions her organization’s vision to power a cultural transformation that normalizes end-of-life conversations so people feel empowered to make choices reflecting their values and beliefs.  She enjoys spending time at the beach and the Poconos with her three children.

Susan GuySusan Guy | Chief Operating Officer, Canine Partners for Life

Susan Guy has been the Chief Operating Officer of Canine Partners for Life since 2014 although her career with the organization began in 2004.  Starting out as the Administrative Assistant, supporting multiple positions, along with volunteering in various capacities, Susan has personal experience with many of the operations of the organization. Her primary responsibilities as COO include the development and management of CPL’s Strategic Plan, creating and editing policies and procedures, oversight of the business department, graduate support, and applicant processing. Susan ensures that all operations are safe and satisfy the highest standards set by CPL’s accreditation with Assistance Dogs International. Perhaps most importantly, Susan continually ensures that all activities promote and support CPL’s mission. She is a creative, positive personality and her heart is truly dedicated to the individuals whose lives are enhanced by the dedication and love of a canine partner.

Kat LuebkeKat Luebke | Community Health Integration Program Director, YMCA of Delaware

Kat Luebke has been with the YMCA of Delaware for over 10 years, currently serving as the Community Health Integration Program Director. She has a background in health and wellness with degrees in Exercise Science and Allied Health. Prior to her current roll as program director, she served as a lifestyle coach for the YMCA’s Diabetes Prevention Program. She is passionate about helping live healthier lifestyles and reducing their risks for chronic diseases.

Kimi MooreKimi Moore | Extension Educator, University of Delaware Cooperative Extension

Kimi Moore is a Family and Consumer Science Extension Educator with the University of Delaware Cooperative Extension. She is the Kent County SNAP-ed Coordinator and works in the area of nutrition and volunteer management. She is a Registered Dietitian with a Master’s degree in Health Promotion. Kimi co-teaches Dining with Diabetes classes throughout the state.

kathleen splaneKathleen Splane, MS | Extension Educator, University of Delaware Cooperative Extension

Kathleen Splane is an Extension Educator with the University of Delaware Cooperative Extension. She works in the area of nutrition, food safety and volunteer management. She has a Bachelor’s degree in Nutrition and Consumer Sciences and a Master’s degree in Health Promotion. Kathleen has been co-teaching Dining with Diabetes classes throughout the state for 18 years.

Dr. SubramanianDr. Ashok Subramanian| CEO and CMO, CarePortMD

Dr. Subramanian is the CEO and CMO of CarePortMD, a startup focused on empowering patients with better access to affordable, coordinated health care. He completed his residency in Emergency Medicine at Detroit Receiving Hospital, serving as chief resident in 1999, where he was a two-time recipient of the Academic Achievement Award and Outstanding Medical Student Teacher of the Year. He spent the next year as a fellow in the NIH-funded lab of Blaine C. White, MD studying the molecular pathology of brain injury following cardiac arrest and reperfusion. While doing basic research, he developed an interest in 3D animation and began to appreciate its utility in helping explain complex biomedical concepts to doctors, patients and investors. Dr. Subramanian was an early advocate of the Internet as a medium to facilitate curation and controlled distribution of information to improve efficiency in health care delivery and research, being one of the first to publish these concepts in the Emergency Medicine literature in the mid 1990s. This confluence of interests led him to his first startup, Animedix, which over the next 8 years provided medical animations and interactive programming for many of the largest pharmaceutical companies as well as biotech companies navigating clinical trials. Over years of observing first-hand the progressive deterioration of the healthcare system he recognized that the most glaring malfunction was a departure of focus from the two most important components of healthcare- patients and doctors. This realization, combined with years of accumulated experience in acute care, pharmaceutical development and Health IT were catalysts for him founding CarePortMD, a company focused on leveraging telemedicine with a proprietary logistics platform to improve access for people to lower cost, better-coordinated health care for chronic disease patients. Dr. Subramanian lives in Landenberg, PA with his wife, Terra and their 6 children and a cantankerous Siberian Husky named Loki.
